The Fitter Futures Warwickshire service is no longer responsible for the referral management of Physical Activity on Referral. All referrals into the Physical Activity on Referral service will now need to go straight to the leisure centre providing the Physical Activity on Referral service.

For referrals to Everyone Active sites, if you do not already have the direct referral link then please contact the Everyone Active team here and they will give you the direct onwards secure referral link Request A Call Back From Our GP Exercise Referral Team. Bedworth Leisure Centre – Bedworth
Stratford Leisure Centre – Stratford-upon-Avon
The Greig Leisure Centre – Alcester

Newbold Comyn Leisure Centre – Leamington Spa
Queens Diamond Jubilee Centre – Rugby

North Warwickshire sites including Coleshill, Polesworth, Atherstone Leisure Centres
There is currently no option to refer into these leisure centres for Physical Activity on Referral. Please refer to other suitable leisure centres
